Self-Reporting Forms

Thank you for participating in the Manitowoc Police Department's self-reporting program. Being the victim of any crime is an unpleasant experience. By allowing the Manitowoc Police Department to respond to your loss in this self-reporting manner, our neighborhood patrol officers will be better able to focus their efforts on crime prevention endeavors and those emergency calls for service which require an immediate on-scene response.

• Statement Form - This form is to be used for official Manitowoc Police Reports. Your written statement will be forwarded to the investigating officer of your complaint.
• Non-Consent/Permission Statement Form - This form is to be used for official Manitowoc Police Reports. Your written statement will be forwarded to the investigating officer of your complaint.

• Bicycle Registration Form - This form is a convenient way to obtain the necessary bicycle information and bring to the Manitowoc Police Department to register your bicycle. The cost is $2.00. For more bicycle information: Manitowoc Bicycle Regulations.
• Alarm Application & Permit - A permit is required for each alarm system on premises within the City of Manitowoc which alarm system terminates to the Manitowoc Police Department dispatch, or any other point of termination which in turn notifies the City of Manitowoc Police Department to respond to an emergency. There shall be a charge of $15.00 for such permit.
